Objective:To introduce the experience of robotic surgery training course in the University of Pittsburgh Medical Center.Methods:The specific process of the training course was introduced in detail, and the results of the training course were summarized and analyzed. A total of 15 surgeons completed the first step of the training (operation table simulation training), 14 (93.3%) completed the second step (pancreatoduodenectomy tissue anastomosis simulation training), and 196 cases of tissue anastomosis simulation training were completed. Statistical analysis was performed using STATA and SPSS software. Wilcoxon rank sum test was used to analyze the data.Results:The average scores of the surface validity Likert scale for anastomosis simulation training were: (15/20) for Pancreaticojejunostomy (PJ), (15/20) for Gastrojejunostomy (GJ), and (17/20) for Hepatocholangiojejunostomy (HJ). In the PJ and GJ simulation training, compared with the gold standard, the error rate and OSATS score were significantly improved ( P > 0.05), and the operation time was not significantly reduced ( P < 0.05). In HJ simulation training, the error rate, OSATS score and operation time did not show significant improvement ( P < 0.05). However, it could be concluded that all the simulation operations performed by the trainees after the training were better than the first time, when linear regression and quadratic fitting models were performed to analyze continuous trainings of the trainees as a group. Conclusion:The training course carried out by the University of Pittsburgh Medical Center is valid and effective, and it can be used for reference to explore a way for the development of robotic surgery fitting to the real conditions of our country.

RESUMEN Antecedentes: los sistemas de salud del mundo se han visto afectados en su lucha contra el COVID-19, generando efectos negativos tanto en la actividad asistencial como en la formación de los residentes. Suspender cirugías electivas, disminuir la participación de residentes en quirófano y otros cambios estructurales de los hospitales determinó que los residentes de cirugía experimenten un marcado déficit educacional en su formación. Material y métodos: modalidad observacional y transversal. Se realizó una encuesta anónima online de 20 preguntas a residentes de cirugía de la Argentina. Se confeccionó una base de datos para el análisis estadístico. Se valoraron variables categóricas y numéricas. Resultados: se recibieron 100 respuestas de la encuesta en la que se identificó predominio de residentes de instituciones públicas. Se calculó un descenso del 63% en la cantidad de cirugías en las que participaron los residentes durante la pandemia. El 77% usó plataformas virtuales para remediar el impacto en las instancias de formación académica. El 57% de los encuestados cuentan con programas de simulación quirúrgica en su hospital. La mayoría experimentó consecuencias negativas en sus habilidades quirúrgicas y en su formación durante la pandemia, pero se identificaron diferencias entre el grupo que dispone de simulación quirúrgica y el que no. Conclusión: para resolver el déficit educativo que generó la pandemia por COVID-19, y como medida para potenciar el aprendizaje de habilidades quirúrgicas en situaciones normales, este estudio recomendaría contar con programas de simulación quirúrgica y fomentar el uso de plataformas virtuales como herramienta de formación académica.


ABSTRACT Background: Health systems worldwide have been affected in their fight against COVID-19, generating negative effects on both healthcare activity and training of residents. Cancellation of elective surgeries, less participation of residents in the operating room and other structural changes in the hospitals resulted in an educational gap in the training of residents in surgery. Material and methods: We conducted an observational and cross-sectional study. A 20-question online survey was conducted among residents in surgery from Argentina. A database was created for statistical analysis of categorical and continuous variables. Results: The survey was responded by 100 residents in surgery; most of them belonged to public institutions. There was a 63% decrease in the number of surgeries in which the residents participated during the pandemic. Seventy-seven percent used virtual platforms to mitigate the impact on academic training and 57% count with surgical simulation programs in their hospitals. Most of them experienced negative consequences on their surgical skills and training during the pandemic, but there were differences identified between the group with and without surgical simulation programs. Conclusion: The availability of surgical simulation programs and the use of virtual platforms as an academic training tool could solve the educational gap generated by the COVID-19 pandemic and enhance the learning of surgical skills under normal conditions.

RESUMEN Antecedentes: El Hospital Eva Perón de la ciudad de Granadero Baigorria se destinó a la atención casi exclusiva de pacientes afectados por COVID-19, lo que implicó modificar las actividades que allí se desarrollan. Allí se realizan actividades correspondientes al Posgrado de Cirugía General de la Facultad de Ciencias Médicas de la Universidad Nacional de Rosario. El objetivo es describir los cambios y el funcionamiento del posgrado de Cirugía General en el HEEP durante la pandemia por COVID-19, y el impacto que esta tuvo sobre la formación de los alumnos. Material y métodos: se realizó un trabajo descriptivo, comparativo. Período 20 de marzo de 2020 al 30 de septiembre de 2020 y el mismo período del año 2019. Variables analizadas: número de cirugías, horas en el hospital, número de guardias, actividad en consultorio y pacientes evaluados, seminarios teóricos, cursado de la carrera de posgrado. Resultados: las cirugías totales se redujeron un 74,88%. Las cirugías programadas se redujeron un 85,59%. Las cirugías de guardia se redujeron un 63,19%. Las guardias de R1, R2 y R3 se vieron disminuidas, al contrario de R4. Las horas en el hospital se redujeron en todos los alumnos. Los pacientes ingresados disminuyeron el 74,06%. La atención en todos los consultorios se vio reducida. Las actividades académicas se incrementaron, de forma no presencial. Conclusión: la pandemia por COVID-19 afectó significativamente la formación de los alumnos del posgrado de Cirugía General del HEEP. Se recurrió a métodos no tradicionales de enseñanza para realizar actualizaciones y discutir trabajos científicos.


ABSTRACT Background: Hospital Eva Perón in the city of Granadero Baigorria was almost exclusively dedicated to the care of COVID-19 patients; thereby, it was necessary to modify its activities, as those of the postgraduate program in General Surgery of Facultad de Ciencias Médicas, Universidad Nacional de Rosario, which take place in this hospital. The aim of this study is to describe the changes made and the performance of the postgraduate program in general surgery at HEEP during the COVID-19 pandemic, and its impact on trainees' education. Material and methods: We conducted a descriptive study comparing the period between March 20, 2020, and September 30, 2020, with the same period in 2019. The variables analyzed included number of surgeries, hours spent in hospital, number of in-house call shifts, activities in the outpatient clinic and evaluation of patients, theoretical seminars, attendance to classes of the postgraduate programs and research work. Results: The total number of surgeries decreased by 74.88%, scheduled surgeries by 85.59% and emergency surgeries by 63.19%. The numbers of in-house call shifts of PGY-1, PGY-2 and PGY-3 residents decreased but not those of PGY-4 residents. The hours spent in hospital decreased in all the trainees. The number of patients hospitalized decreased by 74.06% and there was a reduction in all the activities of the outpatient clinics. The academic activities, performed non-face-to-face, increased. Conclusion: The COVID-19 pandemic significantly affected trainees' education in the postgraduate program in General Surgery at HEEP. Non-traditional teaching methods were used for updating and discussing scientific papers.

RESUMEN Objetivo: describir las percepciones de los cirujanos en formación durante la fase inicial de la pandemia por COVID-19. Material y métodos: se realizó el análisis transversal de una encuesta de 51 preguntas a médicos en formación de un Servicio de Cirugía General en un hospital de la Argentina. Se abarcaron 5 ámbitos: institucional, asistencial, académico, doméstico y emocional. Resultados: en total 55 médicos completaron la encuesta (91,7%): 35 residentes y 20 becarios de perfeccionamiento; 56,4% eran hombres. El 98,1% consideró suficiente la información brindada por el hospital y el servicio sobre las medidas tomadas con respecto al COVID-19. Si bien el 98,1% consideró satisfactorios los instructivos sobre los equipos de protección personal, el 38% informó inconvenientes en su utilización. El 92,5% de los residentes estuvo de acuerdo con el esquema de guardias implementado. El 94,5% incrementó el tiempo dedicado a actividades académicas. El 75% consideró útil el cronograma de clases virtuales implementado. El 72,7% de los encuestados consideró que contaba con espacios donde manifestar sus preocupaciones. El 60% consideró adecuada la supervisión. El 81,81% manifestó preocupación por perder habilidad manual. El 53% dijo sentirse frustrado y el 54% manifestó temor de contagiar a su familia/pareja. Conclusión: conocer las percepciones de los cirujanos en formación en la fase inicial de la pandemia por COVID-19 permitió detectar oportunidades de mejora e incorporar nuevas estrategias educativas para afrontar las fases siguientes, asegurando el aprendizaje y priorizando su bienestar físico y psicosocial.


ABSTRACT Objective: The aim of this study is to describe the perceptions of surgeons in training during the initial phase of the COVID-19 pandemic. Material and methods: A 51-question survey was conducted among physicians in training from a department of surgery in an Argentine hospital. Five domains were explored: institutional, healthcare, academic, domestic and emotional. Results: The survey was completed by 55 physicians (91.7%): 35 residents and 20 fellows. Among the respondents, 98.1% considered the information provided by the hospital and the department about the measures implemented in the COVID-19 stetting was sufficient. 98.1% considered that the instructions about personal protective equipment were satisfactory, but 38% reported difficulties to use them. 92.5% of the residents agreed with the system implemented for on-call shifts, 94.5% increased the time dedicated to academic activities, 75% considered the virtual class schedule implemented as very useful and 72.72% perceived that they had spaces to express their concerns. The supervision was considered adequate by 60%; 81.81% were worried to lose manual dexterity; 53% felt frustrated and 54% were afraid of infecting their family/partner. Conclusion: Understanding the perceptions of surgeons in training in the initial phase of the COVID-19 pandemic made it possible to detect opportunities for improvement and incorporate modifications to address the following phases, ensuring learning and prioritizing their physical and psychosocial well-being.

Resumen La pandemia por el virus SARS-CoV-2 ha afectado fuertemente los programas de educación quirúrgica. El Capítulo Chileno del American College of Surgeons realizó un webinar para evaluar y discutir los efectos de la pandemia en la educación quirúrgica. Este evento contó con la asistencia de 450 personas de 17 países, incluyendo la participación de destacados docentes del área quirúrgica. Las principales conclusiones de este webinar fueron que la pandemia alteró considerablemente los programas de educación quirúrgica. Un 26% de los residentes se contagiaron, pero la mayoría no precisó cuidados especiales. Las intervenciones quirúrgicas disminuyeron hasta en un 90% en algunos programas. Los residentes debieron asumir el cuidado de pacientes COVID-19. Se han implementado sistemas de enseñanza virtual, como seminarios en línea o webinars, discusión de casos clínicos, videos y simulación. Dichas actividades teóricas, al igual que la simulación quirúrgica fueron evaluadas, mayoritariamente, vía web. Las sociedades científicas han tenido un importante rol en estas actividades. En el futuro, los programas universitarios enfrentarán escenarios con menos pacientes y una reducción de las oportunidades de enseñanza para residentes. Las actividades en línea y simulación adquirirán mayor relevancia. Es posible superar esta crisis como lo han hecho los países desarrollados, comparados con ellos, carecemos de un plan nacional de emergencia en salud, en el que los estudiantes y personal sanitario tengan un rol definido en áreas específicas, con metas concretas. El retorno a la "nueva normalidad" estará lleno de desafíos.


The SARS-CoV-2 pandemic has had an impact in surgical residency programs. The Chilean chapter of the American College of Surgeons organized a webinar to discuss and address the effects of this pandemic on surgical education. This meeting had a virtual attendance of 450 people from 17 countries, including the participation of surgical educators. Conclusions of this webinar were that COVID-19 has strongly affected surgical education programs. Twenty six per cent of residents were infected. Most of them did not need special care. Surgical opportunities have decreased up to 90% in some programs. Residents have had to be involved in managing COVID-19 patients. Changes in surgical education, led to a virtual instruction which includes seminars, webinars, case discussion, videos and simulation. Assessment has been performed of theoretical activities, via web. Simulation tasks also have been evaluated. Scientific societies had a very important role in these activities. In the near future, university programs will face different scenarios in hospitals and clinical centers, with fewer patients and reduced clinical instruction for residents. Online activities and simulation will increase in relevance in years to come. It is possible to overcome this crisis, as some developed countries have already done, compared to them, we lack a national emergency health plan in which medical students, residents, doctors, and all health care providers have a designated role in specific areas with clear goals. The return to "the new normal" will be filled with challenges.

ABSTRACT Background: Pediatric procedures have the difficulty of being performed in reduced spaces. Training in reduced spaces has proven to be different in complexity compared to adult laparoscopic endotrainers. Aim: To develop and validate a new neonatal/reduced-space endotrainer. Methods: The simulator was tested and assessed by users with different skill levels and experience in laparoscopic pediatric surgery through an 8-item questionnaire. Construct validity was determined by evaluating the performance of each subject on nine exercises. Results: A 10.5 x 10 x 18 cm acrylic simulator was created, with an internal working surface of 9 x 9 cm. An HD camera was incorporated, with a 0-180° range of movement. All exercises of a Basic Laparoscopic Training Program were adapted on a scale of 1:0.5 to fit in. From 49 participants, 42 (85.71%) answered the survey; 80.5% considered that the simulator reproduces similar conditions to procedures performed in children under one year of age; 61.1% thought that the simulator represents a difficulty identical to procedures performed in newborns; 73.7% considered that the neonatal simulator is more complicated than the adult simulator. Experts showed significantly better performance in all proposed exercises. Conclusion: The simulator has a high-quality image and design that allows training with basic tasks. The endotrainer permitted to discriminate between these different skill levels and was well evaluated by users with diverse surgical experience.


RESUMO Racional: Os procedimentos pediátricos têm dificuldade de serem realizados em espaços reduzidos. O treinamento nesses espaços provou ser diferente em dificuldade em comparação aos endotrainers laparoscópicos adultos. Objetivo: Desenvolver e validar um novo endotrainer neonatal com espaço reduzido. Métodos: O simulador foi criado, testado e avaliado por usuários com diferentes níveis de habilidade e experiência em cirurgia pediátrica laparoscópica por meio de um questionário de oito itens. A validação do método foi determinada pela avaliação do desempenho de cada participante em nove exercícios. Resultados: Foi criado um simulador acrílico de 10,5 x 10 x 18 cm, com uma superfície de trabalho interna de 9 x 9 cm. Uma câmera HD foi incorporada com faixa de movimento de 0-180°. Todos os exercícios do Programa de Treinamento Laparoscópico Básico foram adaptados em escala de 1:0,5 para se ajustarem. Dos 49 participantes, 42 (85,71%) responderam à pesquisa; 80,5% consideraram que o simulador reproduz condições semelhantes às de procedimentos realizados em crianças menores de um ano; 61,1% consideraram que o simulador representa dificuldade semelhante aos procedimentos realizados em recém-nascidos; 73,7% consideraram que o simulador neonatal é mais difícil que o simulador adulto. Especialistas apresentaram desempenho significativamente melhor em todos os exercícios propostos. Conclusão: O simulador possui imagem de alta qualidade e design que permitem o treinamento com exercícios básicos. O aparelho permitiu discriminar entre os diferentes níveis de habilidade e foi bem avaliado por usuários com experiência cirúrgica diversificada.

INTRODUCCIÓN: Una de las vías de acceso a la cavidad abdominal es la laparotomía media, siendo uno de los riesgos más temidos las complicaciones postoperatorias, las cuales pueden disminuirse con difusión de una adecuada técnica quirúrgica. OBJETIVO: Generar un consenso de expertos con pasos clave para realizar una laparotomía media supra-infraumbilical, a través de la metodología Delphi. MATERIALES Y MÉTODO: Por medio de una revisión de la literatura se extrajo el detalle de 37 pasos considerados en el procedimiento, organizado en secciones de preparación, apertura y cierre, siendo evaluados por un grupo de cirujanos de 7 hospitales docentes de Chile, mediante una encuesta online. RESULTADOS: El consenso se alcanzó en dos rondas, con 28 de los 37 pasos extraídos de la literatura, considerados importantes o muy importantes para la enseñanza de la técnica. Discusión: De los 28 pasos identificados como importantes, existe un alto grado de acuerdo para la preparación y apertura de una laparotomía, a diferencia del cierre en el cual no se observa una preferencia clara entre el cierre en una o varias capas entre el peritoneo y la fascia, debiendo analizarse en cada caso específico. CONCLUSIÓN: La identificación de 28 pasos estandarizados constituye un aporte en los programas de formación de residentes y en el desarrollo de modelos de simulación adecuados para la replicación del procedimiento. El aprendizaje correcto de la técnica basado en evidencia es fundamental para disminuir el riesgo de complicaciones asociadas, especialmente en el postoperatorio.


INTRODUCTION: Traditionally, the main surgical technique to access the abdominal cavity has been midline incision laparotomy, being postoperative complications a main concern. Proper teaching-learning methods could help improving the success of this procedure. AIM: Establish an expert's consensus throughout the Delphi Methodology regarding the main steps that need to be considered in a midline laparotomy. MATERIALS AND METHOD: Literature review was conducted, 37 steps were identified and grouped in 3 sections; preparation of the skin, opening incision, and closure. These steps were evaluated online by surgeons of 7 teaching hospitals in Chile. RESULTS: Consensus was reached in two rounds, 28 of the 37 steps were identified as important or very important for the procedure. DISCUSSION: There is a high level of agreement in steps related to preparation of the skin and opening incision. However, there is not a consensus regarding closure of the peritoneum and fascia in one or more layers, being a case to case decision. CONCLUSIONS: Establishment of 28 standardized steps constitutes a contribution to resident teaching programs and the development of simulation materials as an active learning strategy. Satisfactory learning outcomes have a direct impact in the success of the procedure and a lower rate of complications.

Augmented reality is widely used in aeronautics and is a developing concept within surgery. In this pilot study, we developed an application for use on Google Glass ® optical head-mounted display to train urology residents in how to place an inflatable penile prosthesis. We use the phrase Augmented Reality Assisted Surgery to describe this novel application of augmented reality in the setting of surgery. The application demonstrates the steps of the surgical procedure of inflatable penile prosthesis placement. It also contains software that allows for detection of interest points using a camera feed from the optical head-mounted display to enable faculty to interact with residents during placement of the penile prosthesis. Urology trainees and faculty who volunteered to take part in the study were given time to experience the technology in the operative or perioperative setting and asked to complete a feedback survey. From 30 total participants using a 10-point scale, educational usefulness was rated 8.6, ease of navigation was rated 7.6, likelihood to use was rated 7.4, and distraction in operating room was rated 4.9. When stratified between trainees and faculty, trainees found the technology more educationally useful, and less distracting. Overall, 81% of the participants want this technology in their residency program, and 93% see this technology in the operating room in the future. Further development of this technology is warranted before full release, and further studies are necessary to better characterize the effectiveness of Augmented Reality Assisted Surgery in urologic surgical training.

However, they cannot be used to demonstrate trauma surgery due to the emergency situation and lack of informed consent. The aim of this study was to conduct a live demonstration of trauma surgery with a porcine model to increase educational opportunities in trauma surgery.<br>Methods: Live demonstration was conducted at the Center for Development of Advanced Medical Technology (CDAMtec) , Jichi Medical University, Japan. An experienced trauma surgeon instructed three trainees during a live demonstration using pre-planned injuries in a porcine model. A six-point Likert Scale was used on a written survey to determine the value of the program to the viewers. Free-form written comments were also obtained from the participants. Live images of the surgical field were transmitted to a lecture room by a closed wireless LAN with interactive bidirectional audio capability.<br>Results: Eighty-three participants viewed this live demonstration and completed the questionnaire. Participants were highly satisfied with the live demonstration (mean survey scores: 4.6-5.1/6) , and gave very positive feedback concerning the educational value of this program. Nine free-form comments were submitted, which revealed that the participants felt they could acquire concrete skills for trauma surgery.<br>Discussion: Live demonstrations for trauma surgery using a porcine model are a feasible and effective educational tool to demonstrate technical procedures and non-technical skills, with possible added advantages regarding the ethical considerations of performing a live surgery demonstration.

BACKGROUND AND OBJECTIVES: The extraction of the third molar is an invasive procedure and is potentially traumatic for the patient. Preparatory procedures may be used for the management of pain and emotional responses, resulting in a better recovery process for patients. The aim of the study evaluates the effects of a preparatory procedure using an animated informative video on post-surgical pain responses after third molar extraction on youths during the postoperative follow-up. METHODS: 140 patients undergoing third molar extraction were randomly divided into two groups: control (CG) and experimental (EG). For pain assessment, the short-form McGill pain questionnaire was given once before the procedure and four times after the surgery, and the amount of postoperative analgesic consumption was recorded. The informative video was presented to the EG after the first assessment. For data analysis, ANOVA and Tukey tests (p < 0.05) were used. Data analysis indicated that the EG patients reported reduced pain perception for all pain indexes. RESULTS: Significant between-group differences were observed for Sensory and Affective Rank Pain Indexes (PRI-S and PRI-A, respectively), as well as for Global Assessment, at stages 2, 3 and 4 (p=0.0001; p=0.0027; p=0.0001). For Present Pain Intensity, a significant difference between groups was observed (p=0.0004) at stages 3, 4 and 5. The postoperative consumption of analgesics was lower in the EG (p=0.0001), indicating higher medicine consumption by CG patients. CONCLUSION: The results suggest that the preoperative informative video effectively reduced pain perception and the consumption of analgesics.


JUSTIFICATIVA E OBJETIVOS: A exodontia de terceiros molares é um procedimento invasivo e potencialmente traumático. Procedimentos preparatórios podem ser usados para manuseio da dor e de respostas emocionais, resultando em uma melhor recuperação dos pacientes. O objetivo deste estudo foi avaliar os efeitos de um vídeo de animação informativo em respostas de dor após exodontia de terceiro molar em jovens. MÉTODOS: Neste estudo, 140 pacientes submetidos a exodontia de terceiro molar foram randomizados em dois grupos: controle (GC) e experimental (GE). Para avaliação da dor foi utilizado o Questionário de Dor McGill, forma reduzida. Essa avaliação foi realizada uma vez antes do procedimento cirúrgico e quatro vezes após. Foi também registrada a quantidade de analgésicos consumidos após exodontia. O vídeo informativo foi apresentado ao GE após a primeira avaliação. Foram utilizados os testes ANOVA e Tukey para análise de dados (p<0,05). RESULTADOS: A análise mostrou que o GE relatou diminuição de todos os índices de dor. Diferença significativa entre os grupos foi observada nos Índices de Estimativa de Dor Sensorial e Afetiva (PRI-S e PRI-A), como na Avaliação Global de Experiência de Dor nas fases 2, 3 e 4 (p=0,0001; p=0,0027; p=0,0001). Para a Intensidade de Dor Presente (PPI) foi observada diferença significativa entre os grupos (p=0,0004) nas fases 3, 4 e 5. O consumo pós-operatório de analgésico foi menor no GE (p=0,0001), indicando um maior consumo de medicamento para o GC. CONCLUSÃO: Os resultados sugerem que o vídeo de informação pré-operatório efetivamente reduziu a percepção de dor e o consumo de analgésicos.

Authors made a analysis of current situation and the development perspectives of laparoscopic surgery in Cuba from the experience obtained in a general surgery department. Laparoscopy had a positive effect in surgical activity, as well as the organizing, methodological difficulties and in the training of specialists and residents that nowadays remain and stop the development of the specialty(AU)

The clinical teaching is a key link to the cultivation of clinical ability.The quality of clinical practice will affect directly the students' medical treatment level and their comprehensive quality.Therefore,in the integrated Chinese-western surgical clinical teaching,we have adopted the standard teaching and strict management,and deepen the reform in education,thus cultivating a batch of interns with very good comprehensive quality and increasing the clinical teaching quality.

RESUMO

The following is a report on the 20th annual meeting of the Association for Surgical Education (ASE) which was held in Toronto from May 3 through 6, 2000. Because Dr. Masahiro Tanabe (School of Medicine, Chiba University), Dr. Takao Morita (Saitama Medical School), and I were the first Japanese to attend this meeting, I will introduce the activities of the ASE and report on this meeting. The ASE was founded in 1980, and its members represent more than 160 medical schools and institutions throughout the United States and Canada. The president of the ASE from 2000 to 2001 is Dr. Nicholas Lang of the University of Arkansas. The educational clearinghouse is located at the Southern Illinois University School of Medicine. The activities include organizing the annual meeting, publishing Focus on Surgical Education, the quarterly newsletter and official publication of the ASE, and distributing resources for surgical education. The ASE's website is at www.surgicaleducation.com, and its official journal is the American Journal of Surgery. The 20th annual meeting was held at the Four Seasons Hotel, Toronto, under the organization of Dr. Richard Reznick, the president from 1999 to 2000. The meeting consisted of 18 oral and 19 poster presentations, special lectures, workshops, and panel discussions. The importance of education in endoscopic surgery was emphasized in workshops and presentations. We were greatly impressed by the meeting and enjoyed participating in it. The next meeting will be held in Nashville from March 27 through 31, 2001.
